      <abstract xml:lang="ru" lang-variant="original" lang-source="author">
        <p>Хронический болевой синдром в ортопедии и травматологии представляет собой одну из ведущих медико-социальных проблем, обусловленную высокой распространенностью заболеваний опорно-двигательного аппарата и их значительным вкладом в структуру временной и стойкой утраты трудоспособности. В статье рассмотрены основные причины формирования хронической боли, включая дегенеративно-дистрофические заболевания суставов и позвоночника, последствия травм и хирургических вмешательств, а также нейропатические расстройства. Особое внимание уделено механизмам хронизации боли, роли недостаточного и несвоевременного лечения, а также влиянию психосоциальных факторов на ее течение. Цель исследования – описание современных подходов к купированию хронической боли в травматологии и ортопедии, а также обозначение возможности их применения в клинической практике Казахстана. Проведен систематический обзор по методологии PRISMA. Поиск выполнен в базах PubMed, Scopus, Web of Science, Cochrane Library за 2020–2026 гг. Критерии включения: РКИ, систематические обзоры, когортные исследования по факторам риска, эпидемиологии, механизмам и лечению хронической боли. Из 487 первоначальных записей после скрининга и оценки качества (GRADE, RoB 2, Newcastle–Ottawa) отобрано 23 источника. Подчеркивается важность мультидисциплинарного подхода к лечению хронической боли с участием специалистов различного профиля. Особая роль отводится врачу-анестезиологу как ключевому звену в диагностике, выборе тактики лечения и координации комплексной терапии. Рассматриваются современные подходы к купированию болевого синдрома, включающие медикаментозные, интервенционные и нейромодуляционные методы. Большую роль играет необходимость раннего и комплексного вмешательства для предотвращения хронизации боли и повышения эффективности лечения пациентов травматолого-ортопедического профиля.</p>

        <p>Chronic pain syndrome in orthopedics and traumatology represents one of the leading medical and social problems due to the high prevalence of musculoskeletal disorders and their significant contribution to temporary and permanent disability. The article examines the main causes of chronic pain formation, including degenerative-dystrophic diseases of the joints and spine, consequences of injuries and surgical interventions, as well as neuropathic disorders. Particular attention is paid to the mechanisms of pain chronicity, the role of insufficient and untimely treatment, and the influence of psychosocial factors on its course. The issue of studying is to describe modern approaches to the management of chronic pain in traumatology and orthopedics, as well as to outline the possibilities of their application in clinical practice in Kazakhstan. A systematic review was conducted following PRISMA methodology. The search was performed in PubMed, Scopus, Web of Science, and Cochrane Library from 2020 to 2026. Inclusion criteria: RCTs, systematic reviews, and cohort studies on risk factors, epidemiology, mechanisms, and treatment of chronic pain. From 487 initial records, after screening and quality assessment (GRADE, RoB 2, Newcastle–Ottawa), 23 sources were selected. The importance of a multidisciplinary approach to the treatment of chronic pain involving specialists from various fields is emphasized. A special role is assigned to the anesthesiologist as a key figure in diagnosing the source of pain, selecting optimal treatment strategies, and coordinating comprehensive therapy. Modern approaches to pain management are discussed, including pharmacological, interventional, and neuromodulation techniques. Early and comprehensive intervention is highlighted as essential for preventing pain chronicity and improving treatment outcomes in patients with orthopedic and trauma-related conditions.</p>
